The rules of the road.
These terms cover your use of this website. The professional services we provide as a licensed pharmacy are governed separately by your relationship with our pharmacists and by applicable pharmacy law.

1. Acceptance
By using protabpharmacy.com (the “Site”) you agree to these Terms of Service. If you do not agree, please do not use the Site. You may always contact us by phone or in person instead.
2. Not medical advice
Information on this Site is provided for general informational purposes and does not constitute medical advice, diagnosis, or treatment. It is not a substitute for consultation with a licensed pharmacist, physician, or other qualified healthcare professional. Never disregard professional medical advice or delay seeking it because of something you read on this Site. If you believe you are experiencing a medical emergency, call 911 immediately.
3. Forms you submit
When you submit a form on this Site — including the contact form, prescription transfer request, new-patient registration, or physician referral — you confirm that the information you provide is accurate and that you are authorized to share it. Submitting a form does not create a pharmacist–patient relationship by itself; we will follow up to confirm the request and to gather any additional information we need.

Forms you submit through this website do not guarantee medication availability, insurance coverage, prescription acceptance, transfer completion, delivery eligibility, or delivery timing. Prescription services are subject to pharmacy review and applicable law, and certain medications may require a new prescription.
4. Acceptable use
You agree not to use the Site to:
- Submit false or misleading information, including forms on behalf of someone else without their authorization.
- Attempt to access non-public parts of the Site, including our staff admin portal.
- Probe, scan, or test the vulnerability of the Site or attempt to breach any security or authentication measure.
- Send automated or bulk requests, including via scripts or bots, except as expressly permitted (for example, well-behaved search-engine crawlers).
- Use the Site in any manner that could damage, disable, or impair its operation.
5. Prescription services
Our pharmacy services — including dispensing, transfers, and delivery — are provided under the authority of our pharmacy license and applicable Illinois and federal law. The terms of those services are not governed by this document; they are governed by your relationship with our pharmacists, your prescriber's instructions, and the requirements of applicable law (including the patient counseling and recordkeeping obligations of the Illinois Pharmacy Practice Act).
6. Intellectual property
The Site's design, text, graphics, and the ProTab Pharmacy name and logo are owned by us or our licensors. You may view and print pages for personal, non-commercial use. Any other use — including reproduction, modification, distribution, or republication — requires our prior written permission.
7. Third-party links and services
The Site may link to third-party websites (for example, Google Maps for directions, our social-media profiles, or insurance carriers' sites). We are not responsible for the content, policies, or practices of those third parties. Visiting them is at your own risk.
8. Disclaimer
The Site is provided “as is” and “as available.” To the fullest extent permitted by law, we disclaim all warranties of any kind, express or implied, including but not limited to warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Site will be uninterrupted, error-free, or free of viruses or other harmful components.
9. Limitation of liability
To the fullest extent permitted by law, ProTab Pharmacy and its owners, employees, and contractors will not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of the Site, even if we have been advised of the possibility of such damages. Nothing in these terms limits any liability that cannot be limited by law, including liability for personal injury caused by our negligence or for fraud.
10. Governing law
These terms are governed by the laws of the State of Illinois, without regard to its conflict-of-laws principles. Any dispute arising out of or relating to these terms or the Site will be brought in the state or federal courts located in Cook County, Illinois.
11. Changes
We may update these terms from time to time. Material changes will be reflected by updating the “Effective” date at the top of this page. Your continued use of the Site after a change constitutes acceptance of the updated terms.
