We collect only what we need to fill your prescriptions, contact you about your care, and process insurance — and we never sell or rent it.

1. Who we are
ProTab Pharmacy is an independent neighborhood pharmacy located at 8549 S. Cicero Ave., Chicago, IL 60652. This policy describes how we handle personal information you provide through this website and through your relationship with us as a patient.
2. What we collect
When you use this website we may collect:
- Contact information — your name, email, phone number, and mailing address, when you submit a form (contact, prescription transfer, new-patient registration, or physician referral).
- Health information — current and prior pharmacy details, prescription names, allergies, and other clinical information you share to help us provide care.
- Insurance information — your insurance carrier, member ID, group number, and (if you upload one) a photograph or scan of your insurance card.
- Technical information — your IP address, browser type, and basic usage data collected by our hosting provider for security and reliability. We do not run third-party advertising trackers on this site. We use security tools such as Cloudflare Turnstile to help protect our forms from automated abuse. These tools may process limited technical information (such as IP address and browser data) to distinguish human visitors from bots. This technical information, along with the contact, health, and insurance information above, may also be processed by the third-party service providers described in Section 4 (Who we share it with) — for example, our website hosting, database, and email-delivery providers — solely to support our services.
3. How we use it
We use your information only to:
- Fill, transfer, and deliver your prescriptions.
- Verify insurance coverage and process claims.
- Contact you about your prescriptions, refills, and pickup or delivery.
- Respond to messages you send through our contact form.
- Comply with federal and Illinois pharmacy laws and recordkeeping requirements.
We do not sell, rent, or trade your personal or health information to third parties for marketing.
4. Who we share it with
We share information only as needed to provide your care or as required by law, specifically with:
- Your prescribers and other pharmacies, when you authorize a transfer or ask us to coordinate care.
- Your insurance carrier and pharmacy benefit manager, to verify coverage and submit claims.
- Service providers who help us operate (for example, our website hosting, database, and email-delivery providers). These providers are contractually limited to using your information only to support our services.
- Government agencies or law enforcement, when required by court order, subpoena, or applicable federal or state law.
5. How we protect it
We use administrative, technical, and physical safeguards designed to protect the information submitted through our website. Where the federal Health Insurance Portability and Accountability Act (HIPAA) and Illinois law govern your pharmacy records, we handle that information in accordance with those requirements. Concretely:
- All web traffic to this site is encrypted in transit (HTTPS / TLS).
- Insurance card uploads are stored in a private, access-controlled storage bucket and are only accessible to authorized pharmacy staff via signed, time-limited links.
- Access to our admin portal is restricted to authenticated staff with explicit role assignments.
- Bot-protection (Cloudflare Turnstile) is enabled on all public forms to reduce abuse.
6. How long we keep it
We retain prescription and pharmacy records for the period required by Illinois pharmacy law (currently five years from the date of the last prescription activity). Contact form messages and non-patient inquiries are retained for up to two years for follow-up and internal quality review, after which they are deleted.
7. Your choices
You may:
- Ask us what we have on file about you and request a copy of your pharmacy record.
- Ask us to correct inaccurate personal information.
- Ask us to delete non-essential records (we cannot delete prescription records still subject to mandatory retention).
- Opt out of non-essential communications at any time by replying or by calling the pharmacy.

8. Children
Our website and pharmacy services are intended for adults and minors with parent or guardian involvement. We do not knowingly collect information from children under 13 without parental consent.
9. Changes to this policy
We may update this policy from time to time. Material changes will be reflected by updating the “Effective” date at the top of this page. We encourage you to review it periodically.
